Diabetic Shoes

If you have diabetes and Medicare, you may qualify for diabetic shoes.  Patients who have diabetes and at least one of the following are eligible: peripheral neuropathy with evidence of callus formation, history of pre-ulcerative callus, history of foot ulcer, poor circulation, foot deformity, partial or complete amputation.  Our process involves two appointments after the prescription order is received, first a fitting appointment then a dispensing appointment when your custom ordered shoes arrive.  Shoes must be ordered by the physician that oversees your diabetes care.  See order form below.

Diabetes Self-Management Education

Our accredited DSME program offers support and training to people in all stages and types of diabetes.  The goals of DSME are to support informed decision-making, self-care behaviors, problem-solving and active collaboration with the health care team and to improve clinical outcomes, health status, and quality of life.  DSME is covered by Medicare and many other insurances, referral form required.  We will empower you to take control of your diabetes!

Continuous Glucose Monitor Training

In recent years a number of continuous glucose monitors (CGM) have become available to help people living with diabetes monitor their blood sugar more closely for better overall blood sugar control.  If you are new to CGM, the technology can be daunting.  Schedule an appointment with our diabetes educator for training on how to use your device and how to understand the data and reports it provides.  We can help you use this data to troubleshoot high and low blood sugars for better overall control and improved quality of life.
